A growing home hospice organization is seeking a compassionate Registered Nurse to provide patient-centered hospice care in the home setting.
Responsibilities
- Deliver skilled nursing care to hospice patients in their homes through comprehensive assessment, planning, implementation, and evaluation.
- Ensure care is provided in accordance with physician orders, regulatory requirements, and established standards of practice.
- Complete nursing assessments, medication reconciliation, and collaborate with physicians regarding patient care plans.
- Monitor patient progress and update care plans when changes in condition occur.
- Document clinical findings, interventions, and outcomes accurately and within required timelines.
- Educate patients and caregivers regarding disease processes, medications, symptom management, and hospice care plans.
- Coordinate care with physicians, interdisciplinary team members, home health aides, and other healthcare professionals.
- Communicate patient status changes and care needs promptly to appropriate team members.
- Supervise and evaluate home health aide services as required by regulations.
- Assist with care transitions and ensure continuity of care.
- Maintain productivity standards and comply with payer authorization requirements.
- Promote safe, effective, and ethical patient care practices.
Requirements
- Associate Degree in Nursing or Diploma in Nursing.
- Minimum of six (6) months of Registered Nurse experience OR one (1) year of LPN experience.
- Experience in home health, hospice, or community-based care preferred.
- Basic computer proficiency and documentation skills.
- Active New Jersey Registered Nurse (RN) license.
- Current AHA Basic Life Support (BLS) for Healthcare Providers certification.
