A leading home healthcare organization is seeking a Registered Nurse to provide comprehensive nursing care to patients in their homes throughout Monmouth County.
Responsibilities
- Conduct patient assessments and develop individualized care plans.
- Deliver skilled nursing care in accordance with physician orders, agency policies, and professional standards.
- Monitor patient progress and adjust care plans as needed to support optimal outcomes.
- Complete medication reconciliation and collaborate with physicians regarding treatment plans.
- Ensure accurate, timely, and compliant clinical documentation.
- Educate patients and caregivers using appropriate teaching methods and document educational interventions.
- Coordinate care with physicians, case managers, therapists, home health aides, and other healthcare professionals.
- Monitor authorization requirements and maintain compliance with payer guidelines.
- Report patient concerns, changes in condition, and care issues promptly to appropriate personnel.
- Supervise home health aides and complete required supervisory visits within regulatory guidelines.
- Participate in patient care transitions and communicate care plans effectively among team members.
- Maintain productivity expectations and complete assignments independently and accurately.
- Stay current with nursing standards, clinical guidelines, and best practices.
Requirements
- Associate Degree in Nursing (ADN) or Diploma in Nursing.
- Minimum of six (6) months of Registered Nurse experience OR at least one (1) year of Licensed Practical Nurse experience.
- Experience in home health or hospice care preferred.
- Basic computer proficiency and documentation skills.
- Active New Jersey Registered Nurse (RN) license.
- Current AHA Basic Life Support (BLS) for Healthcare Providers certification.
